summary: The Temporary Bilingual Case Manager for the Family Services- Scatter-site Shelter Program will provide case management to homeless families in a scattered site shelter model and will work within a team approach to monitor and support efforts of program participants to obtain community resources and services, improve living skills and gain permanent housing.



  • Using Motivational Interviewing and a collaborative approach, conduct in-depth assessment to best determine the immediate client concerns and/or barriers to housing, including, but not limited to: financial, legal, mental health, substance abuse, employment, health care and housekeeping
  • In collaboration with heads of households, develop a Re-Housing Plan (RHP) for each family member over 18 years of age. The RHP must include specific goals in the areas of housing, education and training, financial management, employment and career development; Meet with families, at least bi-weekly, to review RHP progress and plan next steps
  • Document and track all families’ meetings, interactions and information in the ETO (Efforts To Outcomes) database. This includes, but is not limited to, HUD Assessments, RHP, Case Notes, Validations and Clients Lists
  • Manage a case load of 15 to 19 families
  • Complete a monthly unit inspection for each family assigned
  • Satisfactorily complete quarterly client-files audit performed by program managers and/or Assistant Director of Family Services
  • Serve as liaison and collaborate with Heading Home’s programs, making sure families fully participate and benefit from these programs’ services. Successfully refer all new families to the Economic Mobility Center and Placement Program for their initial assessments
  • Obtain resources and services as requested and/or needed to address barriers to housing as stated above including but not limited to; CORI mitigation, credit repair, and assistance with financial applications
  • Ensure that all clients receive an explanation and assistance with navigating housing application processes and referrals to all appropriate housing opportunities; including but not limited to, local housing authorities, regional non-profit housing agencies, private management companies, and community development corporations
  • Develop partnerships and collaborative relationships with internal and external service providers to ensure clients receive comprehensive services
  • Participate in the on call weekly rotation schedule
  • Participate in individual and group supervision as assigned by Program Manager
  • Participate in all required training and professional development activities
  • Work collaboratively with Development staff to participate in volunteer activities as needed
  • Perform other tasks as requested by Program Manager, Assistant Director of Family Services and/or Director of Family Services
  • Observe all program and agency rules and policies


  • Associate’s Degree in Social Work (or related field)
  • Bilingual


  • Case management experience


  • Experience with crisis intervention regarding issues such as homelessness, mental health, substance abuse and other housing barriers


  • A valid Massachusetts driver’s license in good standing and a car
